Scholarship Guidelines, Instructions and Application Form

Each year the Foundation awards scholarship grants to graduating seniors.  These grants are awarded to seniors whose parents income prohibits them from qualifying for a government grant or whose Grade Point Average(GPA) is not high enough to receive an academic scholarship.  Through this grant program, the Foudation seeks opportunities where a relatively modest amount of money can make a significant difference in a childs educational opportunites and life.  

Currently, an amount of $250.00 is allocated for 15 students per semester annually attending a four year college and $250.00 annually for 15 students attending a two year college.  Application deadlines for grants from this program currently is June 15.  

Scholarship Grant Qualifications.  To be eligible for a grant from this program, a student must meet three basic criteria:

1.  Verification of parents income
2.  Maintain a 2.5 GPA or better for two consecutive semesters (ie. junior/senior; senior/senior year)
3.  Registered full-time with 12-15 credit hours at an accredited two year or four year college

Application Procedure.  All requests for grants from Change-A-Life Foundation must utilize the attached form, which describes in detail the needed information.  Care should be taken to insure that each item of the required information is provided fully, clearly and concise.  Because multiple copies of the form, cover letter, official school transcripts, and attachments will be made for review by the individual members of the Foundation’s Scholarship Grant Committee, applicants are reminded to refrain from submitting unnecessary, duplicative or promotional information.  Submission of booklets, videotapes, computer discs and other such media should be specifically avoided.   
 

Change-A-Life Fund  -  Through this fund, you can do your charitable giving.  Start a Change-A-Life fund with a minimum gift of $5000 and a commitment to maintain at least $1000 in the fund.  You may make fully deductible contributions to your fund at your convenience.  You may also make recommendations for scholarships to students who reside in enterprise zone areas.

Angel Giver  -  Many people, visionary people, understand the value of changing the lives of our children through philanthropy.  They love and faithfully give their regular support to better the lives of others and in turn create a better world for us all.  Our Angel givers give substantial amounts of money to the Foundation monthly, quarterly and even annually on a regular basis.  Thank you for your ongoing support.

Associate Giver  -  As an associate giver, you can join us in our efforts to change the lives of children throughout generations.  Our associate givers give on a monthly basis and are dedicated to our cause. 

Corporate Sponsor  -  Our corporate sponsors realizes the importance of securing our future by investing in the lives of our children today.  They along with us are dedicated to the mission of positively changing the life today of as many children as we can to ensure a secure future for all of mankind tomorrow.   Our Corporate sponsors pledge their money, goods and services as ongoing support of the Foundation.

Include the Foundation in your Will  - You can establish a fund with Change-A-Life Foundation with a bequest.  You could not leave a more beneficial or enduring legacy to our future for generations to come.

Include the Foundation in your Estate Planning  - Start a fund by naming the foundation as the beneficiary of a life insurance policy or by making the foundation the beneficiary of a deferred gift, such as a charitable remainder trust or a charitable lead trust.

Transfer an existing Private Foundation  -  Sometimes the expense and time it takes to manage a private foundation become burdensome.  You may transfer that foundation to Change-A-Life, preserving its name and its charitable purposes for all time.  Your private foundation suddenly has all the advantages of a fully tax-deductible charitable fund and none of the disadvantages of IRS regulations over private foundations.

Non Cash Contributions - You can donate items of value to the Foundation that we will in turn auction off.  (Example:  automobile, jewelry, furs, paintings, antiques, etc.)  All proceeds received from the auctioned items will go towards the mission of the foundation.  A tax deductible receipt (fair market value) for the auctioned item will be issued to the donor. 

Funding Priority  -  If either of the above fore mentioned ways of giving do not meet your need to give, simply choose one of the four funding priorities of Change-A-Life (Change-A-Life Life Line, Change-A-Life Reads, Change-A-Life Scholarship or Change-A-Life Works.)  Or you can just support our day to day operations. 

As an ongoing effort to support the Foundation, we have several items available for purchase including Change-A-Life Art Superstore and Change-A-Life Super Mall.  All proceeds that the Foundation receives from any purchase from either the mall or the art store will go to the Foundation to support our mission to the children.
