Scholarship Guidelines, Instructions and Application Form

Each year the Foundation awards scholarship grants to graduating seniors.  These grants are awarded to seniors whose parents income prohibits them from qualifying for a government grant or whose Grade Point Average(GPA) is not high enough to receive an academic scholarship.  Through this grant program, the Foudation seeks opportunities where a relatively modest amount of money can make a significant difference in a childs educational opportunites and life.  

Currently, an amount of $250.00 is allocated for 15 students per semester annually attending a four year college and $250.00 annually for 15 students attending a two year college.  Application deadlines for grants from this program currently is June 15.  

Scholarship Grant Qualifications.  To be eligible for a grant from this program, a student must meet three basic criteria:

1.  Verification of parents income
2.  Maintain a 2.5 GPA or better for two consecutive semesters (ie. junior/senior; senior/senior year)
3.  Registered full-time with 12-15 credit hours at an accredited two year or four year college

Application Procedure.  All requests for grants from Change-A-Life Foundation must utilize the attached form, which describes in detail the needed information.  Care should be taken to insure that each item of the required information is provided fully, clearly and concise.  Because multiple copies of the form, cover letter, official school transcripts, and attachments will be made for review by the individual members of the Foundation’s Scholarship Grant Committee, applicants are reminded to refrain from submitting unnecessary, duplicative or promotional information.  Submission of booklets, videotapes, computer discs and other such media should be specifically avoided.
